When There’s No Pathway, You Have to Build One

Rare biomarker testing, patient identification and precision medicine commercialization, a first-in-class oncology launch story.

This particular gene fusion affects fewer than 1% of solid tumor patients, scattered thin across a small number of tumor types. When the client launched a first-in-class therapy targeting it, the commercial challenge wasn't finding patients on an existing pathway. There was no pathway.

NO COMMERCIAL INFRASTRUCTURE
In NSCLC, an established testing culture existed, but nothing was guiding labs or HCPs toward NRG1 specifically. In pancreatic cancer, there was no model for stakeholder engagement at all, a biomarker with no clinical precedent in either setting. 
NO CLINICAL AWARENESS
Most physicians had never considered this biomarker clinically actionable. Even where it appeared in a report, it went largely unrecognized.
POSITIVE PATIENTS, HARD TO FIND IN TIME

Even where a positive result existed, the patient was one of a handful scattered across the US. Finding the patient at the clinical decision point was its own problem, separate from testing altogether.

WRONG TESTING METHODOLOGY
Standard DNA-based NGS cannot reliably detect this class of gene fusion. RNA-inclusive NGS is required, and without it, real positive patients were quietly being missed.

74,820 patients were tested across the program in 16 months, achieving a 100% testing rate in one of the rarest biomarker populations in oncology. Testing volumes also grew by *19.6% in the most recent six months versus the first six months, showing that Diaceutics did not simply deliver a one-time testing milestone, but built a program that continues to identify patients and generate value as the market matures.
